What you need to know
You have two ways to file claims: cashless or reimbursement.
Cashless process
Planned hospital admission: Hospitalisation is planned in advance for procedures such as cataract surgery. Authorisation is sent 7 days before the date of admission.
Emergency admission: When hospitalisation is unexpected due to an emergency, including RTA or stroke, a request for authorisation must be sent within 24 hours of hospitalisation.
Reimbursement process
File your claim through the reimbursement process if you choose a hospital that’s not part of the ICICI Lombard network of hospitals or for OPD or AYUSH treatment.
Be sure to keep all original bills, reports, prescriptions, and discharge summary from the hospital.
When you’re ready to file your claim:
- Complete and sign your claim form
- Provide full details of hospitalisation
- Send all documents to ICICI Lombard Health Care so they arrive within 21 days from the discharge date
Claims are processed and approved or denied within 15 days from the date of receipt of all required documents.
You must include the following supporting original documents with your claim form:
Pre- or post-hospitalisation
- Itemised bills
- Payment receipts
- Investigation reports
- Prescription bills
- Discharge summary
- Original or copy of your cancelled cheque and PAN Card of proposer or primary member for EFT payment
OPD treatment (for dental and vision, too)
- Treatment summary
- Vision card
- Doctor consultation report
- Payment receipts
- Investigative reports
- Discharge summary
- Copy of Health card, original cancelled cheque, and PAN Card for EFT payment
AYUSH treatment
- Discharge summary
- Treatment summary
- Doctor consultation report
- Itemised hospital bill, along with any interim bills
- Payment receipts
- Investigative reports (original lab results, X-rays, sonogram, CT scans, MRI, etc.)
- Prescription bills
- Detailed bills with supporting prescriptions from the treating doctor for all miscellaneous charges
- Copy of Health card, original cancelled cheque, and PAN Card for EFT payment

Appropriate use of company benefits
Misuse of company programs, including the submission of false bills, is a serious matter that can have significant legal and financial implications for both the individual involved and the company.
